Vital Records Certificates

Birth Records

  • Certified copies of birth records are available for individuals born in Adams or whose parents resided in Adams at the time of their birth.
  • Not all vital records are public records. Some birth records are restricted.
  • If you are not certain whether the record of birth is on file in Adams, please call the Clerk's Office at (413) 743-8300 Ext. 176 before ordering online.
  • Request for Birth Certificate Online.
  • Request Birth Certificate by Mail.

Death Records

  • The Adams Clerk's Office issues certified copies of death records of any person who died in Adams or if the person was a resident of Adams at the time of their death and died in another community in Massachusetts.
